Beyond Malnutrition Diagnosis in the Hospital: Comprehensive and Patient Centered Nutrition Care Post Discharge

Description

Malnutrition is widespread and often underdiagnosed. With a prevalence of 20-50% of patients admitted, but only up to 8% identified, addressing this challenge necessitates a comprehensive approach extending beyond hospitalization. Effective care requires meticulous discharge planning that not only recognizes the presence of malnutrition but also leverages available community resources. This session will begin with a brief review of updates from the CDR Chair to inform credentialed nutrition and dietetics practitioners of publicly available resources and information to aid in practice and continuing education. The session aims to illuminate the critical significance of post-hospitalization malnutrition care while fostering enhanced patient health outcomes. By delving into the rationale behind malnutrition-focused programs post-discharge, the discussion will underscore the imperative need for a seamless continuum of care. In addition, it will explore real-world case study examples to exemplify existing programs that can be seamlessly integrated into community settings, thereby facilitating effective and sustained malnutrition care beyond the hospital walls.

  • Explore successful options to integrate malnutrition treatment during the in-hospital discharge planning and post-discharge care.
  • Review the key stakeholders in the transitions of care and development of programs that support malnutrition care through discussion of successful hospital and community programs.
  • Formulate suggested processes and procedures to apply during the discharge process in order to address gaps in malnutrition care.

    Skip has been serving as the Clinical Outreach Dietitian at New Hanover Regional Medical Center since Jan 2019. He previously spent 3 years as the Nutrition Director for Campus Dining at East Carolina University. As a graduate of the University of North Carolina at Wilmington, Skip was very excited for an opportunity to give back to a community that he loves. As the Clinical Outreach Dietitian, Skip now provides a mix of telehealth, counseling, home care, and community care coordination to a wide variety of patients in Wilmington and the surrounding areas.

    Pediatric Clinical Dietitian, Program Coordinator/Education Specialist, UCSF Benioff Children’s Hospital, UC Berkeley

    Alicia Henson is a pediatric clinical dietitian at UCSF Benioff Children’s Hospital in Oakland, California. She has over eight years of experience in clinical practice, currently caring for children in the inpatient and outpatient setting. She specializes in caring for young patients with fatty liver disease and celiac disease. She also teaches the Healthy Together Nutrition class at the Children’s Hospital, a class that aims to promote a healthy lifestyle for children and their families. Outside of work, she serves as an executive committee member of the Vegetarian Nutrition Dietetic Practice Group and is actively involved in volunteering for medical missions and nutrition education projects focused on preventing chronic diseases globally.

    Maria Zamarripa is a registered dietitian for the VA Eastern Colorado Healthcare System. She is an Integrative and Functional Nutrition Certified Practitioner and is also boardcertified specialist in gerontological nutrition. She provides medical nutrition therapy in the home care setting and leads quality improvement projects and research initiatives to promote quality of life and optimal health for older adult Veterans across the continuum of care. Maria has a MS in Nutrition and Food Sciences, a BS in Food Science and Human Nutrition. She has been a Registered Dietitian for over 7 years, during which time she has produced peer-reviewed publications as well as consumer publications. In addition, Maria serves in the VA Marketing and Nutrition Informatics Subcommittee.

    Anne Coltman is the Senior Director of Quality, Standards, and Interoperability for the Commission on Dietetic Registration. Anne has previous experience as a nutrition support dietitian in surgical critical care, a clinical nutrition leader, and a hospital operations senior leader with a wide variety of experience implementing malnutrition initiatives with a focus on quality. She is a Lean Six Sigma Black Belt and a Fellow of both the Academy of Nutrition and Dietetics and the American College of Healthcare Executives.
